New Patient Registration
Register Currently Capped
We regret to inform you that, we will be undertaking a temporary list suspension. This means that we will not be registering new patients at our practice.
This decision has been taken to enable the maximising of care for our current patients at a time of a national GP shortage.
We will continue to register:
- newborn or adopted babies of parents already registered at the Practice who are still living within our boundary.
- first degree relatives of patients already registered at the Practice who are living at the same address as registered patients and are still living within our boundary.
For assistance with finding a GP Surgery in Sussex to register with, please contact the NHS Sussex GP Allocations Teams by email sxicb.gpallocations@nhs.net stating the full names of everyone who needs to register, and your full address, including postcode or you can call 0300 140 9855. The team will provide you with the available practice options and arrange for an allocation if necessary.

How to Register
To register as a Patient with this Practice you need to be living within the Pound Hill, Worth and Copthorne areas as shown on the map held at each surgery. Please ask at reception if you are unsure whether you fall within our catchment area. You can see our catchment area below and check your postcode.
You must:
- Either provide a medical card with details of your new address in the space provided and a contact number or complete the purple registration form (GMS1). You must fill out the whole form. If you are unsure, we have examples at reception to help.
Also requested:
- A utility/credit card bill stating your local address and some form of photo ID to allow us to sign you up to the online services. In case of a family, one person's ID will suffice unless the surname of the adults are different.
- A completed New Patient Questionnaire.
If you need to see a doctor before you can provide this information, you will be directed to the local GP walk-in Service at Crawley Hospital who will be able to help.
If you are on any repeat medication you must make an appointment to see a GP.
